We collect information you provide directly, including your name, contact details, appointment preferences, medical/dental history, HMO details, and any messages you send us through our booking form or contact form. We do not collect information from this website beyond what you submit through these forms.
3. How We Use Your Information
Your information is used to schedule and confirm appointments, prepare treatment plans and fee quotations, process HMO claims, coordinate referrals, send appointment reminders and recall notices, and respond to your inquiries. We do not sell patient information to third parties.
4. Data Sharing
We share information with HMO providers strictly to process your benefit claims, and with referring dentists only regarding the case they referred. We do not share your data with any other third party except where required by law.
5. Data Retention & Security
Patient records are retained for as long as necessary to provide continuing care and to comply with applicable record-keeping regulations, and are stored using reasonable administrative and technical safeguards.
